Getting Started
The
downloadable forms on this page are meant to assist our families in easily
obtaining the forms necessary to commence this process. In no way are
they meant as a substitute for one-on-one interaction with our genetic
counselors and doctors. Before you complete any of these forms, it is
imperative that you have had at least one telephone consultation with
our genetic counselor. Once we receive your completed application and
mutation report, you will be scheduled for a conference call with one
of our doctors to review the procedure, answer questions and discuss next
steps.
At any time in the process, we invite you to contact us with questions,
concerns or comments. We recognize you are making an emotional, psychological
and financial decision if you decide to use in vitro fertilization (IVF)
and PGD to help you have a healthy baby.
